The Resource Navigator is a full-time position which functions in the coordination and management of supportive services and resource navigation for guests of Community Care Fellowship. This position will primarily work the daytime shift (Est. 7:00 AM – 3:00 PM, Mon. – Sat.) within our Rapid Re-Housing program in North Nashville at the Highroad Place. The Resource Navigator reports directly to the Associate Director. The Resource Navigator should be aligned with Community Care Fellowship’s mission and core values. The Resource Navigator is specifically responsible for these functions:

• Guest Services
o Connect guests with outside agencies
o Prepare Homeless Letter Verifications as needed
Provide guidance and referrals and assist guests with expressed needs

• Records, Meetings, & Administration
o Maintain and prepare annual statistics of services provided
o Enter and maintain guest’s records in HMIS and CCF’s SalesForce database
o Assisting with other staff tasks as needed, contributions to social media content, occasional help with fundraising efforts, engagement with volunteers and supporters, and a general spirit of gratitude, teamwork, and collaboration
o Attend Board meetings and Nashville-Davidson Continuum of Care meetings as needed

• Resource Navigation
o Creatively networking resources to open up opportunities for housing and critical services
o Coordinate services and advocate for the guests with other agencies and service providers
o Making contact and building relationships with people experiencing homelessness while providing resources, referrals, and support
o Navigating complex social services with people so they can obtain needed resources like identification documents, healthcare, food assistance, transportation, housing, and other needed services and supplies
o Help navigate people to move from encampments into permanent housing utilizing the Coordinated Entry System through HMIS
• Program Coordination & Support
o Work with other Resource Navigators from CCF and People Loving Nashville to coordinate the Rapid Re-Housing program in North Nashville through tenant referrals from the Metro Homeless Impact Division
o Work with the Associate Director and other staff to coordinate the CCF Mobile Navigation Center program by identifying, enrolling, and supporting guests through resource and housing navigation
o Support the Employment & Education Manager in coordinating the CCF Career Counseling Program by scheduling counseling appointments, helping guests complete necessary paperwork, and helping guests find long-term sustainable employment that leads to self-sufficiency

• Training
o Complete trainings as needed for continuing education and professional growth, such as:
 Housing Navigator Training (Metro Homeless Impact Division) – required
 HMIS Coordinated Entry Training (Metro Homeless Impact Division) – required
