Clergy Benefits Online Payments
Individual benefit participants may use this link to make payments online for Health Plan (dependents or Optional Continuation), and for Dental Insurance.
You can make a one-time payment from your checking account (ACH) without creating a profile. Or, you can create a Profile so the system will remember who you, and will pre-fill the form the next time you make an entry. In the profile, you can also instruct the system to make recurring payments (every month) if you wish.

UMR EZ Claim Form for submitting a health plan claim directly to UMR when the health care provider does not submit a claim. (Do not use this form for drug or dental claims.)
Caremark Paper Claim Form for submitting a prescription drug claim directly to Caremark when the pharmacy is not in the network and does not submit a claim. Claims can also be submitted online by registering here. (Do not use this form for medical or dental claims.)
Dental Benefit Options Two voluntary dental plans offered to clergy and lay employees eligible for the Tennessee Conference Health Plan. Both dental plans offer a 15-20% discount of certain vision services if you select a “Vision Service Provider.” Premiums are the responsibility of the individual and are not supported through the Annual Conference budget.

Designation of Beneficiary – Wespath Administered Plans
Contribution Election Form to begin or change your UMPIP before-tax or after-tax contributions. The employee and salary-paying unit must complete the agreement. The employer/salary-paying unit should keep the original form for its payroll records. Please make a copy of the completed form to Wespath Benefits and Investment at the address on the second page of the form, or you may fax it to 1-847-866-5195.

UMPIP Plan Sponsor Adoption Agreement (instructions and forms to set up a church to handle its pastor’s and staff voluntary pension contributions to UMPIP. Including making the church a “Plan Sponsor” with Wespath and the form for the participant to set or change the amount the church withholds from their salary and pays into their account at Wespath).
Rental/Housing Allowance Resolution This resolution, which is adopted each year at annual conference, allows clergy to exclude from their taxable income benefits they receive from Wespath, up to the amount allowed by law.
