Golden Cross Foundation Grant / Counseling for Senior Adults

 

Introduction In April 2018, The Golden Cross Foundation of the Tennessee Annual Conference of the
United Methodist Church awarded Belle Meade United Methodist Church a grant of $2500 to
help provide counseling for adults 65 years & older. Beginning May 1, 2018 (or when the
church has physically received the funds from the Foundation), these funds will be available to
counseling clients from both the congregation and the broader United Methodist community
who cannot afford the full fee for counseling at the Counseling Center at Belle Meade United
Methodist Church. This form outlines an understanding of how grant funds will be distributed
from the church to the counseling center.

Funds Distribution
To maximize the funds, individual contributions will be limited to $50 per session. When
potential clients who are 65 or older present for counseling and request financial assistance,
the use of grant funds will be considered.
The potential client must be unable to pay a minimum of $100 for counseling services. The
potential client will be asked to complete the counseling center’s request for financial
assistance form and must provide proof of income. The counseling center’s sliding scale will
be applied to these clients based on information provided. If a client cannot afford even the
minimum fee on the fee scale, assistance will be provided, and the grant funds will apply.
If the potential client can only pay a fee that is less than the minimum scaled fee, the church
will provide a maximum assistance of $50 per session and the counseling center agrees to
accept the client’s payment plus the church’s supplement as payment in full for all counseling
sessions for that client.
If a potential client can pay the scaled fee, the church will provide up to the maximum
assistance of $50 per session, but the total of the client payment and the supplement will not
exceed $100 and the counseling center will accept the client’s payment plus the church’s
supplement as payment in full for all counseling sessions for that client.
The counseling center will submit requests for grant funds anonymously and will provide a
count of hours provide and the amount of supplement to the church with the monthly session
summary reports. The church will provide the counseling center funds for sessions provided
until all funds have been distributed.

Summary & Examples

 The church will use grant funds from the Golden Cross Foundation Grant to supplement
the fee for counseling sessions for adults 65 or older who cannot afford a minimum of
$100 per session.
 The counseling center will have potential clients complete a request for assistance and
provide proof of income.
 A client who cannot afford the minimum scaled fee of $50 will be able to pay what he or
she can afford, the church will supplement to a maximum of $50 per session and the
counseling center will accept the combination of client payment as supplement as
payment in full for the sessions.
Example: A client presents for counseling and through the application process is
determined to only be able to pay $25 per session for counseling. The church will
supplement no more than $50 for this client’s sessions and the counseling center will
accept the client payment plus the supplement ($75) as payment in full for this client’s
sessions.
 For a client who can afford the center’s scaled fee, the church will provide up to a
maximum of $50 per session of counseling, but the combination of the client payment
and the church supplement will not exceed $100 per session. The center will accept
this as payment in full for the sessions.
Example: A client presents for counseling and through the application process is
determined to be able to pay the counseling center’s scaled fee for their income and
household level, let’s say $75. The church would supplement only $25 for this session and
the counseling center will accept the total of the client payment and supplement ($100) as
payment in full for the sessions. (If the client’s scaled fee was $50, the church would
supplement the maximum $50 and the total for session would still be $100.)
 The counseling center will provide anonymous information on the number of sessions
and amount of supplement in a report with the monthly session summary provided with
the center’s rent check.
