2011 Benefits Open Enrollment Package - For most clergy, open enrollment is during the month of July.  Completed forms for health plan, dental, or pension benefits should be mailed to Administrative Services, 304 S. Perimeter Park Dr. Suite 4, Nashville TN  37211.

Tennessee Conference Health Plan

Benefits at a Glance (2010) - Summary of health benefits provided to clergy by the Tennessee Conference; provided.  (This document does not include optional dental, life, or disability coverages.)

Health Plan Summary Plan Document (SPD) as amended 1-1-2011 - Booklet that defines all the health benefits provided by the Tennessee Conference for full time clergy appointed to serve local churches, and other plan participants.

Report of Conference Health Plan Committee (2011) - as approved by Annual Conference 2011. Includes Schedule of Eligibility and Health Plan Premiums for 2011.

Privacy Policy for Health Plan

Health Plan Claim Form - for submitting a health plan claim directly to UMR when the health care provider does not submit a claim.  (Do not use this form for drug or dental claims.)

Prescription Drug Claim Form - for submitting a prescription drug claim directly to Caremark when the pharmacy is not in the network and does not submit a claim.  Claims can also be submitted online by registering at https://www.caremark.com/wps/portal.  (Do not use this form for medical or dental claims.)

Disability and Death Benefits (Comprehensive Protection Plan)

CPP Summary of Benefits - One page summary of clergy disability and life insurance.

CPP Summary Plan Description (SPD)- Details of the disability and death benefits provided to clergy.

CPP Beneficiary Change Form - Complete this form any time you want to change your beneficiary designation. You may want to review your designation each time there is a change in your family status, such as birth, marriage, divorce or death.

Tennessee Conference Retirement Benefits

CRSP at a glance - One page summary of Clergy Retirement Security Program which took effect 1/1/2007.

CRSP and UMPIP Beneficiary Change Form - Complete this form any time you want to change your beneficiary designation. You may want to review your designation each time there is a change in your family status, such as birth, marriage, divorce or death.

UMPIP Before- or After-Tax Contributions Agreement – Complete this form to begin or change your UMPIP Before-tax or after-tax contributions.  The employee and salary-paying unit must complete a Before-tax and After-tax Contributions Agreement. This form does not need to be returned to the General Board of Pension and Health Benefits (General Board). The employer/salary-paying unit should keep the original form for its payroll records.
 
UMPIP Billing Change Form – Complete this form to begin or change to your UMPIP before-tax or after-tax billing amount. This form should be returned to the General Board of Pension and Health Benefits.

Report of Conference Board of Pensions (2010) - approved by the Tennessee Conference June 14, 2010.  Includes Funding Plan for Pre-82 Plan.

Report of Conference Board of Pensions (2009) - approved by the Tennessee Conference June 2009.

Housing Resolution for Clergy who are Retired or Disabled - This resolution, which is adopted each year at annual conference, allows clergy to exclude from their taxable income benefits they receive from GBOPHB, up to the amount allowed by law.
